59 of the Worst-Rated Nursing Homes in Philadelphia and New Jersey

When you’re looking for a nursing home for your parent or grandparent, you want to ensure they’ll receive nothing but the best of care and never need PA nursing home lawyers.  That means steering clear of nursing homes that have earned poor ratings for issues like understaffing, health violations, and low-quality service for residents.  Using Medicare Compare ratings, Philadelphia nursing home abuse lawyer Brent Wieand has compiled 59 of the lowest-ranked nursing homes within 25 miles of Philadelphia, including facilities in New Jersey.

The nursing homes listed below have received very low scores from Medicare Nursing Home Compare, a tool provided on the federal government’s Medicare website.  All of the facilities featured on the list are located in, or within 25 miles of, Philadelphia.  All have received overall ratings of one or two stars, out of five possible stars.  Two stars means “below average,” while one star is “much below average.”

One asterisk indicates that the nursing home is located in Philadelphia.  Two asterisks means it is located in New Jersey, within 25 miles of the city.  Otherwise, all homes are located in Pennsylvania, also within 25 miles of Philadelphia.

Federal fines have been indicated where facilities were penalized during the past three years.  Several facilities have been fined on multiple occasions.

    • Laurel Square was fined $6,013 on December 4, 2014.
  10. Majestic Center for Rehabilitation and Sub-Acute Care**
  11. Majestic Oaks Rehabilitation and Nursing Center
  12. Maplewood Nursing and Rehabilitation Center*
  13. Meadowview Rehabilitation and Nursing Center
    • Meadowview was fined $152,523 on June 12, 2015.
    • This facility was previously fined $52,650 on December 9, 2014.
  14. Mount Laurel Center for Rehabilitation and Healthcare**
    • Mount Laurel was fined $20,150 on April 21, 2015.
    • This facility has also been identified as a “Special Focus Facility,” which means it has “had a history of serious quality issues” as determined by the Centers for Medicare and Medicaid Services.
